For Insurance physicals, a set form is usually provided by the insurance company. Please email or fax this to us so that we can prepare test items in advance. We will call you to schedule the appointment. In some cases, the insurance company sends us the form, items requested and payment authorization letter in advance. However, if we have not received the letter, we will ask you to kindly settle the account upon completion of the examination.
