Information necessary to book

To make an appointment, we require the following information:

  • If you have started your visa application and have an NZHR number, please provide us with this number.
  • If you do not have an NZHR number, please tell us which type of tests (INZ 1007/1201/1096) you will need to do. The tests will differ depending on the type of visa you are applying for and the instructions you have been given for your application process.

Unfortunately we cannot advise on which tests you need for your visa application, as it varies according to individual circumstances.

Items to Bring

  1. 1) Original valid passport
  2. 2) Physical Examination Forms (INZ 1096 and/ or INZ 1007 or INZ 1201) – please complete Section A of these forms prior to your appointment. See below for links to these forms. Photos will be taken on-site so it is not necessary to attach these.
  3. 3) Proof of pregnancy (medical certificate/ mother-baby book with expected delivery date) for pregnant or possibly pregnant applicants.
  4. 4) Applicants with history of TB or serious medical condition(s) should bring a medical report with detailed information noting treatment and/or prognosis in English from their treating doctor.
  5. 5) “Mother and Child’s book” (if available) for children 6 years old and under.
  6. 6) Glasses or contact lenses (if worn). Corrected vision must be better than 0.5, 20/40 or 6/12 in the better eye.
  7. 7) Vaccination records, including for COVID-19 (if available)

** Please bring printed hard copies of all proofs/ documents (e.g. medical certificates, vaccination records) Digital files, photos, or apps cannot be accepted **

Precautions

  • Examinations are performed by appointment only
  • Fasting is not required for this examination
  • Photos will be taken on site
  • The examination will take approximately 2 hours.
  • Applicants will be called in order of appointment time (not in the order of arrival or check in)
  • Please come at the set appointment time. Examinations cannot be started early for those who come early. Please avoid coming in more than 30 min before your appointment time.
  • Female (INZ 1007) applicants: please avoid coming in during menstruation. Applicants who are menstruating during their examination may be asked to come back on another day or reschedule their appointment
  • Please inform our staff if you are pregnant. Pregnant patients are exempt from Chest Xray testing.
  • Persons under 18 years of age must be accompanied by a parent or guardian.

Chest Xray

When taking the chest Xray, you will be asked to remove all clothing covering the upper body (including underwear). Please avoid clothing that cannot be taken off quickly.

Examination Results

All results will be submitted electronically directly to New Zealand Immigration via the e-medical system. The result will be sent within 5-7 working days (not including Saturdays/ Sundays/holidays). If there is an abnormal finding in your examination, we will contact you as soon as possible.

Our office is open from 9am to 5.30pm Monday to Friday, and 9am to 1pm Saturday.

When you arrive at the clinic, please check in directly at the 2nd floor Physical Examinations Room (Room 15).
