1) PATIENTS ATTENDING FOR FLU SHOTS ON SATURDAY MORNINGS

In order to avoid congestion in the waiting room, all patients attending for flu shots on Saturday mornings must have a PRE-BOOKED time slot and must PRE-PAY via credit card. Credit card payments must be completed at least 24 hours prior to your appointment time. Please do not come to the clinic until your scheduled time. Upon arrival at the clinic, please go directly to room number 15, NOT to the main reception area. We are unable to accept government issued vouchers for flu vaccination on Saturdays. Patients are requested to complete the following forms (print and complete) BEFORE coming to the clinic:

Form A: Flu shot information sheet (ONE PER PERSON)
Form B: Declaration of current health status (ONE PER FAMILY) – please check your temperature before coming to the clinic.

 

2) PATIENTS ATTENDING FOR FLU SHOTS ON WEEKDAYS

On weekdays, we also request that you PRE-BOOK for flu shots. It would help us to reduce congestion if you also pre-pay via credit card, but this is not an absolute requirement on a weekday. Check-in will be at the usual main reception desk. We will accept government issued vouchers for flu vaccination on weekdays only. The following 2 forms will need to be completed (regardless of whether you are using the voucher from the government or not). Again, completing these before coming to the clinic will help to ease congestion:

Form A: Flu shot information sheet (ONE PER PERSON)
Form B: Declaration of current health status (ONE PER FAMILY) – please check your temperature before coming to the clinic.

NOTES: Children born from 2nd April 2005 may receive a government issued voucher for flu vaccination in the mail. This voucher will give you a 3,000 JPY discount (NOT a free vaccination). You will need to complete a separate government issued form on arrival at the clinic so please be prepared for some delay.
